The Hubitality program understands that a hospitality worker in crisis needs assistance as quickly as possible.

To hasten the application process, we’ve compiled a list of financial and crisis incident-related documentation to help you complete your application.

Our goal is to remove as many barriers as possible to receiving emergency assistance while maintaining the integrity of our program and protecting the mission of our organization. Injury

Slip, fall, crash at work or home? Support for an injured hospitality employee missing work for an extended period of time. 

INCIDENT DOCUMENTATION:

  • Medical verification (such as a signed doctor’s note), including diagnosis and expected time missed from work
  • Medical receipts, if applicable
  • Accident or injury report if possible (example: police report from car crash)

FINANCIAL DOCUMENTATION:

  • Current pay stub
  • Lease agreement
  • Current utility bill(s): water, gas and electricity; if utilities are bundled with lease agreement, please supply documentation

*We verify employment and incident with your employer.
**Employee must be able to show financial need.

Please note: we do not pay medical expenses.

Illness

Cough, fever or something more serious? Support for a food service worker battling an illness and missing work for an extended period of time. 

INCIDENT DOCUMENTATION:

  • Medical verification (such as a signed doctor’s note), including diagnosis and expected time missed from work
  • Medical receipts, if applicable

FINANCIAL DOCUMENTATION:

  • Current pay stub
  • Lease agreement
  • Current utility bill(s): water, gas and electricity; if utilities are bundled with lease agreement, please supply documentation

Disaster

Flood, fire, or storm? If you have housing issues because of a disaster, GK can help keep you on your feet and a roof over your head.

INCIDENT DOCUMENTATION:

  • Incident report from emergency service agency, such as fire department

FINANCIAL DOCUMENTATION:

  • Current pay stub
  • New lease agreement
  • Previous lease agreement
  • Current utility bill(s): water, gas and electricity; if utilities are bundled with lease agreement, please supply documentation.

Funeral

Support for a food service worker who has lost a member of their immediate family (parent, grandparent, sibling, grandchild or child) or for the family of a deceased food service worker.

INCIDENT DOCUMENTATION:

  • Death certificate
  • Documentation verifying relationship to the deceased
  • Accident report, if applicable
  • Verification of funeral service (for example: funeral program)

FINANCIAL DOCUMENTATION:

  • Current pay stub
  • Lease agreement (if work missed)
  • Current utility bill(s): water, gas and electricity (if work missed); if utilities are bundled with lease agreement, please supply documentation
  • Receipts from funeral expenses
  • Receipts from funeral travel expenses
